US Coast Guard Launches Highest Level of Investigation into Titan Submersible Loss

The U.S. Coast Guard said it has initiated a Marine Board of Investigation (MBI) following the tragic incident involving the Titan submersible and its five occupants. The crew of the Polar Prince research vessel lost communication with the Titan submersible during its dive on June 18, 2023. After an extensive search and rescue operation, the wreckage of the submersible was discovered on the ocean floor, approximately 500 meters off the bow of the Titanic.

The MBI, known as the highest level of investigation within the Coast Guard, aims to determine the cause of the casualty and shed light on any contributing factors. According to the US Coast Guard, upon completion of the investigation, the Board will present its findings, including the collected evidence, established facts, conclusions, and recommendations, to the Commandant.

Key areas of focus for the MBI include investigating potential acts of misconduct, incompetence, negligence, unskillfulness, or willful violation of the law by licensed individuals, as well as by Coast Guard personnel or other individuals involved. The Board will also assess if civil penalties or criminal acts under U.S. laws have occurred and if there is a need for new regulations or amendments to existing laws to prevent similar incidents in the future.

The Coast Guard MBI is working closely with national and international authorities, such as the United States National Transportation Safety Board, Canadian Transportation Safety Board, French Marine Casualties Investigation Board, and United Kingdom Marine Accident Investigation Branch, according to the US Coast Guard, to ensure a comprehensive and collaborative investigation.
